How do I request my User ID and password for the online claims system if I didn't receive one | If you have not received your login information to access the online claims system, you can request access by filling out our online form. |
What is Multi-Factor authentication (MFA)? | Multi-Factor Authentication is a security measure that requires users who utilize EyeMed's online claims system to verify their identity through dual authentication steps before gaining access to the online claims system.
Each user within your practice that utilizes the online claims system is required to have their own unique account to access the system. Usernames and passwords should never be shared within your practice. Each individual account must have its own unique email, user ID and password to protect patient data. How do I log in to the online claims system using Multi-Factor Authentication (MFA)? | Using your unique online claims system user account, you will log in to the online claims system. From there, if you don't have a mobile number associated with your account, an email with your token code will be sent to your email address on file to enter on the screen.
If you have both a mobile phone number and an email associated with your account, a new page will appear where you can select whether to receive your token code via text or email. You can use our Multi-Factor Authentication Claims System Process job aid for step-by-step instructions. |
How do I register for a network lab? | You're able to register with the in-network lab of your choice in our online claims system prior to placing the first order. Our Registering for Network Labs job aid walks you through the lab registration process. |

How can members find me? | All participating providers will appear on our provider locator, helping to ensure members understand which office(s) are part of their network. The provider locator can be viewed here. |

How do I submit claims for medically necessary contact lenses? | You will have to use a custom claim form to submit medically necessary contact lenses. Your Provider Manual will provide more information once it becomes available. |
How do I update my location information? | You can update your location information through the online claims system. Here you can term providers from a location or term a location itself. You can also update a locations business hours, address, phone number, online scheduling information and the location email address. You can use our Updating Provider Information job aid for step-by-step instructions. |
How do I add a new location or provider, change TIN/owner, and change or update payment information? | You can submit a change of information request by filling out our online form. |
