For Providers

If you would like EyeMed to provide an update on the Provider Locator reflecting changes to your practice, please follow the below process to update your provider profile:

  1. For office hours, please visit our online claims system and click the Manage my Profile tab. Then navigate to Hours of Operation and make your changes.
  2. If your practice is not in use at this time and you are no longer able to serve members, please notify us by completing a Change/Move Location Address form. Indicate in the notes how your practice has been affected.
